With massive weight loss, in a short period of time, patients are often left with a significant amount of excess skin.  Because of the rapid weight loss, the patient’s skin doesn’t “bounce back” adequately, resulting in a multitude of problems including problems with moisture, skin break down, hygiene and body image problems (aesthetic concerns).  This excess skin remains a reminder of the person’s past problem with obesity and limits the patient’s satisfaction with the weight loss he/she has achieved.  Although patients are often very pleased with the improvement in their medical issues (such as diabetes and high blood pressure) achieved after weight loss, they are still not happy when they look in the mirror.  Plastic surgical procedures after weight loss may help the patient achieve their goals and significantly improve their self-image.  It is important to work with a plastic surgeon with extensive experience due to the fact that treatment of the post-bariatric surgery patient is unique in that the patient’s skin elasticity has been permanently decreased, potentially resulting in faster recurrence of some of the patient’s presenting complaints.  Dr. Pousti has helped many patient’s in this situation.

Many patients complain of “empty, droopy, saggy” breasts after weight loss – in fact, patients often say that they were happier with their breasts before weight loss than after (just like some patients are happier with their breast during pregnancy than after).  Most patients after massive weight loss, benefit from breast augmentation (saline or silicone gel breast implants) along with breast lifting (mastopexy) surgery.  The mastopexy operation serves to raise the nipple-areola complexes higher on the chest wall (by removing redundant or loose skin) while the breast augmentation serves to increase fullness of the breasts especially along the superior pole of the breasts.  Usually, after significant weight loss, both breast implants and breast lifting are required for optimal results.

Saline Implants
Saline implants have a silicone rubber shell that is inflated to the desired size with sterile saline. The implants are placed under the pectoralis muscle deflated and therefore, the incision used for this type of implant is minimal.  These implants can be overfilled to achieve a more rounded appearance. 

Saline implants have some advantages over silicone implants. Silicone implant ruptures are harder to detect. When saline implants rupture, they deflate and the results are seen almost immediately. When silicone implants rupture, the breast often looks and feels the same because the silicone gel may leak into surrounding areas of the breast without a visible difference.  Patients may need an MRI to diagnose a silicone gel rupture.   Saline implants are also less expensive than the silicone gel implants.

Silicone Implants
Silicone implants have a silicone rubber shell that is filled with a fixed amount of silicone gel. Most silicone gel-filled implants are not adjustable and therefore, the incision used to place the silicone gel implant is larger than the incision needed for saline implants. The silicone gel implant cannot be overfilled – it comes in an exact size that cannot be manipulated.
Silicone implants vary in shell surface (smooth/textured), shape, profile, volume, shell thickness, and number of shell lumens.

Some surgeons feel that silicone implants have a more natural look and feel than saline implants because silicone gel has a texture that is similar to breast tissue. Each patient differs in the amount of breast tissue that they have.  If a patient has enough breast tissue to cover the implant, the final result will be similar when comparing saline implants versus silicone gel implants.  If a patient has very low body fat and/or very little breast tissue, the silicone gel implants may provide a more natural result. 

As of 2006, the FDA has approved the use of silicone gel implants manufactured by the Mentor Corporation and Allergan (formerly McGhan) for breast augmentation surgery.

Patients will also often complain of loose skin of the abdomen and/or back areas.  The redundant folds of skin may be “depressing” to patients who have worked hard to drop their weight (and maintain it) only to be left with even looser skin.  Unfortunately, like the issues described above, these areas of excess skin do not improve despite the most rigorous diet and exercise schedule.  Ironically, the redundancy of skin may worsen despite patients’ best efforts.  Abdominoplasty (tummy tuck) surgery serve to remove the excess skin of the abdomen while tightening up the abdominal wall muscles.   This operation serves to improve the contour of the abdominal wall while the patient is standing, leaning over and sitting down.  At the same time, the pubic area (which is often of concern to patients after weight loss) may be reduced and “lifted”.  If the pubic area is not addressed during the abdominoplasty procedure, the patient often will be dissatisfied with the end result of surgery.

 

Often patients will complain of excess skin circumferentially at the level of the abdomen and lower back.  Lower body lifting and belt lipectomy procedures serve to remove redundant skin of the abdomen and back circumferentially – this operation serves to improve the contour of the buttocks, hips, thighs and abdomen.

 

Weight loss can leave loose or “hanging” skin of the upper arm, between the elbow and shoulder.  This area is addressed using the brachioplasty or arm lifting surgery.  One of the keys of this operation is to plan the resulting scar to fall at the lower edge of the arm so that the scar is not visible from the front or back view.  This operation may be extended to address the excess skin of the upper chest as well (upper body lift).

 

Patient often complain of extra skin of the thighs as well.  This area can be treated using the thigh lifting techniques.  A medial thigh lift will address the inner thighs while the lower body lift addresses the outer part of the thighs.

Saline or Silicone: Which Fails More Often?

Breast implant failure is one of the problems that can develop after your breast implants have been placed. Breast augmentation is commonly considered a relatively safe operation.  However the implants used are simply not indestructible and can fail over a period of time.  What are the main causes and how can you avoid this? Saline and silicone breast implants fail in different ways so you should consider the difference between the two before making your final implant selection.

Eventually all implants may simply wear out over time and develop a hole in the outer shell, causing a leak. It naturally happens over time as a result of the motion within the implant wall. A tear may develop in the outer shell due to the pressure of the implant contents due to your natural motion. Many advancements have occurred in the manufacturing of implants increasing the durability, but they are still not indestructible.

From 1992 to 2006 the most common type of implant in the United States was saline since the FDA had withdrawn its approval of silicone. Until November of 2006 saline was the only FDA approved implant, while the silicone implant was used primarily in clinical trials. Most of the implants subject to fatigue and failure today are saline as a result.

Saline implants are subject to deflation because the fluid can easily flow through a hole in the outer shell. Due to the liquid nature of saline, the failure occurs rather rapidly as the fluid leaks through the hole in the outer shell. Women with saline implants have reported that their implants have deflated as little as within a day, while others over a few weeks.

Since the leakage causes deflation, women notice rather quickly that their breast is not as full as it once was.  The breast implant failure is very noticeable. While this is problematic, of course, some women consider this to be an advantage since it is much more difficult to tell if a silicone implant has actually ruptured. Since the failure occurs very quickly with saline, the woman knows she needs to visit with her plastic surgeon quickly.

Silicone gel implants are made of a thick, cohesive gel and are not prone to rapid deflation. The cohesive silicone gel is rather thick and sticky and is bonded together.  Even when cut in half with a knife, the implant holds itself together.  Unless the hole developing in the implant is very large, the gel is not likely to squeeze out. Even with a large hole, while the silicone may bulge out of the tear, it will eventually fall back in due to its thick, viscous consistency.  Therefore silicone breast implant failure is less noticeable and might only be detected during a mammography.

Both of these implants are subject to failure. Since saline implants are subject to rapid deflation, it increases the possibility that the patient will need to replace the implant at some point in her lifetime. In most cases, even upon failure, silicone breast implants will cause no noticeable change in appearance or feel.

Upon evidence of implant failure you will need to visit again with your plastic surgeon. While the rupture of implants are not as common as in the past is remains as one of the important factors you must consider when choosing your implant. When you meet with you plastic surgeon be sure to discuss the benefits and risks of saline and silicone breast implant ruptures.

How can you avoid breast implant failure? Keep these points in mind if you decide to get breast implants:

* All breast implants fail over time.  While cohesive gel implants are considered more durable, the fact is that you will probably have to replace the implants within about ten years.  Age of the implant therefore is a factor in implant failure.

* You should avoid high risk activities which place significan  pressure or compression on the breasts. Simply be careful with high impact and vigorous sporting activities and try not to fall on your chest or receive any trauma to your breasts.

* Your plastic surgeon will give you after-care instructions.  Be sure to follow them! Attend all follow up appointments with your doctor and avoid any contraindicated activities as advised. If you give your breast implants the proper care and attention they deserve they should last a long time.

Breast augmentation can give you the breasts you have always desired. If you choose your plastic surgeon carefully, you can be assured that they will do everything possible to insure excellent results which will be free from any unwanted complications. Chemical Peels as a Non-Surgical Option

People mostly associate cosmetic surgery with major surgical procedures like facelift or nose reshaping. The fact is there are many non-surgical options available to address any level of aesthetic appearance need. Phenol may be contra indicated when changing skin pigmentation is not desirable. The recovery time from a phenol peel is typically the longest.

Breast Augmentation Recovery Time

Breast augmentation surgery is now the leading cosmetic plastic surgery performed in the world today. Recovery from the surgery is less incapacitating that with some other plastic surgeries but nonetheless takes some time for the healing process to occur. As with any surgical  procedure, precautions must be taken and follow instructions from your plastic surgeon closely to minimize the chance of infection and to ensure the muscle and tissue around the breast implants heals correctly.

Saline vs. Silicone Gel Breast Implants

Breast implants are available in two forms: Saline implants and silicone gel implants. Each implant type has pros and cons associated with it. Saline implants are most often used in the United States. The surface of this type of implant may be smooth or textured. The shell of the saline implant is made of a strong silicone and the saline is added to the implant at the time of surgery. The advantage of saline implants is that if the implant bursts, the saline is non-toxic and readily absorbed by the body. Saline breast implants, of course, had been the only choice in the United States from 1992 to 2006.

Some people still consider silicone gel implants to be somewhat controversial. Women have made claims of connective tissue disorders (CTD) and silicone implants are believed by some to be the cause. However, the actual causes of CTD are yet unknown. In 1992, the Food and Drug Administration placed a moratorium on silicone-filled implants. However, in 2006, the FDA approved the use of silicone gel implants. These implants are widely used today for breast augmentation surgery.

Unlike saline implants, silicone gel implants arrive to the surgery center already pre-filled. The newly approved silicone gel implants have three layers of coating to protect the body from silicone gel bleed and prevent ruptures. Due to the pre-filling of the implants, the incision required to insert the device must be larger than with saline implants. Like saline implants, the texture of the surface of saline gel implants may be textured or smooth and are available in different profiles.

Recovery Time

Recovery time after breast augmentation surgery is different for everyone due to the differences of our bodies. However, you may expect to experience quite a bit of discomfort for a few days to a few weeks after the procedure. Size and placement of the breast implants will determine level of discomfort. For example, larger implants will cause more discomfort. The most important thing is that you continue to take prescribed medications, particularly antibiotics, to ward off infection which can be serious. One of the leading causes of death in hospital is from infection. Be sure to keep your body strong after the procedures with plenty of rest, plenty of water and taking all of your prescribed medication.

Immediately after your surgery, your breasts will be on your chest wall, appear swollen and even a bit misshapen. The process of the breast tissue completely healing and the breasts taking on their permanent shape will take approximately six to eight weeks.  It can take several months for the implants to descend into their optimal position.  Stitches will generally dissolve in about one week. However, swelling and bruising may continue for several weeks.

Your doctor will prescribe a compression bra or bandage. Immediately after surgery, compression bandages prevent the build up of fluids. The compression bandage / bra used over the first few weeks after surgery helps to keep the implants in place and helps speed recovery after breast augmentation surgery. It is critical at this time to continue wearing your compression garments throughout your recovery time to prevent rapid descent of the implant.

Tips to Reduce Pain

Your physician will prescribe pain medication for the days following the surgery. Use as prescribed. When the pain has subsided, you may wish to use ibupropin or another over the counter pain reliever. Warm showers will sometimes relieve mild pain. Ask your doctor when he / she recommends that you be able to shower and bathe after surgery.

Recommended Exercises After Surgery

Very mild exercises are recommended for the weeks following breast augmentation surgery. As to be expected, the muscles of the upper torso will be swollen, sore and bruised. For exercise during these times, try a low impact leg workout or a sitting workout. It is important that you do not stretch the surgical area until proper healing has occurred. After about a month, you should be able to resume your normal exercise routine. Be sure you gain clearance from your doctor prior to any regular exercise routine.

Returning to Regular Activities

Full breast augmentation surgery recovery will take at least a few months and will vary with each individual. It can be tempting to speed the process but if you do not follow directions you may interrupt the process and reduce the life expectancy of your new implants.  You may return to regular activities, including your job, after about two weeks recovery time.  Some women have reported a far quicker return to work, but again this varies greatly from one individual to another. Your doctor will provide details regarding using the muscles around your implants and how to minimize the chance of complications from returning to activities too soon.

If you plan appropriately, your recovery time from breast augmentation surgery can go quickly and smoothly. Yes, there is a very real temptation to rush the process, to wear the new exciting bras too soon.  Be patient and be aware of the procedures.  Rushing the process can delay healing and even damage the results. If you treat yourself well during this time you will increase your satisfaction with the procedure over a longer period of time.
