Blepharoplasty
Introduction
Eyes are the mirror to the world. Indeed and eyes speak so much about the face of a person. They give out the inner emotions of the person, even when the person is trying to be someone else. And unfortunately they convey the age of a person, even if that person does not feel that old. Very early age related changes are easily visible in both the upper as well as the lower eyelid. An ageing lower eyelid with its hanging fat bags gives the feeling that the person is actually older than he is. in addition the tear trough deformity that is often present (the hollow crease present below the eyes), further aggravates the situation. 
It is possible surgically to correct this to a large extent, in a process known as blepharoplasty or eyelid rejuvenation surgery. 
Causation of the “ageing eyelids”
The skin of the lids tends to sag as a result of ageing. Further the fat bags which are present over the eyeballs tend to come out of their natural position, and get more visible. In some persons, the eyebrow also tends to sag down, and it is important to hitch the eyebrows as well as correct the eyelid problem.
Treatment
The surgery can be done under local anesthesia in most patients. This involves an injection containing local anesthesia in the skin of the eyelids. The skin incision is just under the eyelashes, and the scar is easily hidden in the lines of the lids, and no evidence remains of the surgery. There is also the approach where the incision is done through the conjunctiva, known as trans-conjunctival blepharoplasty. Then the excess skin over the eyelid and protruding fat bags are removed, to leave behind a younger looking eyelid, and the “sad” look is not present anymore. The fat bags can also be used to cover the tear trough, and reduce the hollowness that is present under the eyes. This procedure has been popularised by Dr Ruth Graf in Brazil, under whom the author has trained . 
Possible problems
The skin over the eyelids is very thin, hence there is easily some swelling that comes there. This can take between 3-6 weeks to disappear. Thus the patient must know, that he/ she will need to wear glasses if they have to go back to work in this period. Some conjunctival inflammation can be there, particularly with the trans-conjunctival approach. If the patient is hypertensive, it is mandatory that this is controlled well before surgery. If there is the use of aspirin or similar drugs, they need to be stopped at least 7 days before surgery, otherwise there is a greater chance of bleeding.

Breast reduction

Large breasts were at one time considered extremely aesthetic and very sociable, but in todays world it is often a source of embarrassment. It also brings with it the problems of ill-fitting clothes, inability to wear modern, designer kind of hep clothing as well as the problem of back pain, bra strap marks on the shoulder, sweating and fungal infection below the breast line, and a general decreased self esteem.

Large breasts may be from teenage itself or more commonly after childbirth and lactation. The major complaint women have after childbirth and feeding is that the breast have now sagged down, and started to look very bad. The only treatment for these problems is surgical reduction known as reduction mammoplasty or breast reduction. No other non surgical procedure is successful, may it be use of ultrasound technology or radio frequency based devices. It should be understood, that there is an absolute increase of breast tissue as well as skin, which needs to tackled, and surgery is the only option

Procedure

Breast reduction involves an actual removal of excess breast tissue as well as skin, and shaping the breast to a more aesthetic and shapely figure as per the pre-operative discussions with the patient. Commonly this involves two types of procedures. One is a wise pattern with inverted ‘T’ scar pattern; the other is vertical short scar pattern. The latter technique is now well established, has a smaller scar, but takes significantly more time to give the final breast shape

The ‘T’ scar technique
This involves a scar on the breast in the shape of an inverted T. The advantages of the technique is
this technique is easier to teach and more predictable results
the final shape is immediately visible after surgery, with little changes in the long term period
there is little potential of scar spreading
The disadvantages of this procedure is
long scar ( though the scar is hidden below the breast, so is visible only on lying down
there is chance of tissue breakdown along the junction of the limbs of the inverted T(this is not common, and though there is rapid healing)

The vertical short scar technique
The procedure involves only a vertical scar without the horizontal component. As a result of which the skin excision is limited to the region around the areola (upper part). This procedure has been popularized by Dr Ruth Graf (Brazil)(with whom the author has trained with)
The advantages of this procedure is
shorter scar
The disadvantages of this procedure is that
more difficult to teach
it takes almost 6 months to show the final breast shape
there may be need to revise the surgery in almost 30% of patients
the skin shaping is as not as good, because lesser skin is removed from the lower half of the breast
chances of scar stretching in the region of the areola

Possible problems with breast reduction

The patient will be required to stay in the hospital for 1 day after the surgery. The possible problems associated with the procedure are
lactation ability is lost on most patients. Ability to feed is the exception in most cases
sensibility of breast tissue will take almost 6 months to recover
chances of asymmetry are there
in large reductions, some tissue loss is possible, however this does not majorly effect the final result
like any procedure, there may be chances of seroma or a hematoma formation

Results

Breast reduction is an extremely satisfying procedure, because the results are predictable to a large extent in the preoperative period. How much tissue needs to be removed is decided, and therefore the patient has a clear picture of what her breasts are going to look like after surgery. There is a need to wear specially designed pressure garments to support the breast tissue for a period of 3 weeks. Breast asymmetries can also be tackled at the time of the surgery

The face is the social parameter of a person. We often judge a person by his or her initial appearance, and it is the face which tells the observer a lot about us, and unfortunately about our age also. The visible effects of ageing appear first on the face, and make the person look older than he or she feels.
Brief physiology of ageing
The face betrays the effects of gravity and the effect that the beating of the sun has on it. There is an effect both on the skin as well as on the underlying structures such as the SMAS ( the underlying soft tissue below the skin lying over the muscles of the face). The ultraviolet radiation has the ill effect of skin damage and gravity adds the effect of descent of tissues. It is therefore essential that any surgical treatment protocol involves tackling both the skin as well as the underlying SMAS layer. It is possible now to surgically peel off a few years off somebody’s face without giving her/ him too many visible scars
The Face Lift - what is it and how is it done
Surgical face lift is a procedure that can be done under local (under injections + sedation) or general anesthesia. It involves an incision at the junction of skin of the face and the ear. (nowdays short scar face lifts are done, where the scar is very small and over a period of time are rarely visible enough to draw attention). After the incision, the skin and the underlying SMAS is tightened and pulled in an antigravity fashion to oppose the effects of gravity. Once the symmetry is achieved on both the sides, the incisions are closed taking care not to disturb the hair line or disturb the appearance of the ear. In men the side burns can get effected and gets closer to the ear, therefore they are forewarned about the possible requirement of laser to allow hair removal. However it is not required in the vast majority of the population
Results
Following a face lift, you can expect a role back of almost a decade in your physical appearance. Combined with a PIXEL laser, it gives also a fresh and rejuvenated, soft skin to complement the effect of surgery. A good facelift, is symmetric, maintains the hair line, both in the region of the temple and in the back of head, as well as maintaining the symmetry of the ear. We can expect the effects of a surgical face lift to last for over a decade to almost 15 years if the procedure has tackled both the skin and the underlying SMAS. A number of surgeons do not tackle the SMAS for fear of injuring the underlying nerves, but no face lift is complete, without this, and at best skin only procedure, gives benefit for a few months
Revision of a face lift is also possible, and it is also possible to correct a prior poor face lift. Most common effect of a poor prior procedure is a disturbed hair line in the region of the temple, but such cases can be tackled with hair transplant in this area. The face lift can be combined with eyelid surgery (blepharoplasty) to give a young look to the eyelids also.
Complications of Face lift
The most common complication is bleeding and haematoma formation in the post operative period. Therefore it is essential that no patient should be on aspirin therapy prior to procedure and blood pressure should be well under control before surgery.
Hospital stay
The patient need only stay in the hospital for about 24-36 hours, after which he or she can go home, but under watchful supervision. There is a definite downtime, and time has to be taken off from work/ social responsibilities to allow the incisions to heal and all the post operative swelling to settle down.

Something about Liposuction

Liposuction
Liposuction continues to the most common plastic surgery procedure all over the world, and also in India. It is basically a permanent removal of fat cells by the use of a suctioning device. It is a safe procedure, with minimum down time, and fast recovery. It has been shrouded in small controversies, and bad results, but this has been due to the procedure being done by untrained personnel, and other medicos who are not Cosmetic Surgeons. So it is important that a potential client look through the profile of their treating physician and asses really whether that person is actually capable of delivering all that he promises. Do not be led away by claims that they can remove 10 kgs or 12 kgs of fat and make you superthin. Please remember that the safe limits of liposuction are between 3-5% of the total body weight.
Procedure
The procedure involves infiltration of fluid into the areas to be liposuctioned. This has multiple benefits. It helps to loosen the fat, reduces the bleeding dramatically, and allows pain control after surgery as well. Usually I would infiltrate fluid in a ratio of 1:1 with the amount of fat that I plan to aspirate, ie, if I feel that I will aspirate about 1000 cc of fat from an area, I would like to infiltrate less than 10000 cc of the fluid in that area. I actually infiltrate much less than that, only about 0.5 :1 ratio. This has the benefit that I never face the risk of fluid overload ( potentially dangerous condition, where there is too much water in the body, and the lungs get filled with water). The other benefit is that, what is aspirated is pure fat, and not water mixed with fat. This gives an exact estimate of the amount of fat taken out. We surgeons have a bad habit of including the amount of fluid aspirated into the total volume taken out also, so that we can boast of having done large liposuctions. What is important is not the amount taken out, but how much of actual fat there is.
The procedure is done with small incisions over selected areas of the body, after first discussing with the patient, and planning the site of the incision. These incisions generally are not visible and heal very well. They are only about ½ cm in length. There may not be a need to admit the patient in the hospital, so that the patient can go home the same day.
Precautions
After a liposuction, there is a need to wear specially designed pressure garment, over the areas liposuctioned to allow better skin redraping. This may take upto 6 weeks. Any anticoagulant use such as aspirin should be stopped 1 week prior to surgery; smoking should also be stopped 2 weeks before to 2 weeks after surgery for best results
Newer technology
Power liposuction, vibro-liposuction, VASER, and laser liposuction are some of the newer innovations in this field.
In Power and Vibro Liposuction the mechanical effort to carry out liposuction is reduced by the use of special machines, which produce a to and fro rapid motion, thus reducing mechanical fatigue and making the procedure faster. However these are significantly more expensive than conventional liposuction
VASER is the use of internal ultrasound device to melt fat. The advantage is that this is significantly faster, and allows the surgeon to work closer to the skin, and thus better skin draping is achieved, and thus better figure control is achieved. However this is a very expensive device, and expects the surgical cost to double if you ask for a VASER.
Laser lipo allows very good redraping action on the skin; however its role in actual fat reduction is a bit controversial. Thus this has not found use in general practice.
Please remember that liposuction is not a weight reducing procedure, but a figure correcting/ or a body contouring procedure

Breast Implant Sizes: Big, Bigger, Biggest

Breast augmentation sizing is a challenge for the doctor as well as the patient. Breast implant sizes affect each woman’s body differently. It is difficult for the doctor to predict precisely whether a 350cc breast implant size will make you a C cup, for example, until they get into the surgery. A 350cc on you may very well look different than on your friend.

Another challenge has to do with a patient’s reference to cup size. Women will often say, “I want to be a D cup”, or “Make me a C cup”. Many doctors will actually refuse to discuss cup size since there are many factors which cup size does not describe. Even more importantly, cup size is not standardized by the bra industry. One manufacturer’s B cup, might be a another manufacturer’s C cup. That makes it difficult for a doctor to say precisely what cup size you will be.

Due to these issues, many doctors shy away from specific discussion of breast implant sizing. Some will offer sizes in very general terms such as “Big”, “Bigger”, and “Biggest”. Like each of these perspectives, this too has advantages. Based upon the doctor’s experience they feel they know what is proportional. Regardless of the value of this, most women want to have more input on the breast implant size decision than this.

Since many doctors have different methods of breast implant sizing, it can be difficult as a patient to know how to communicate what you want. The best way to communicate your desired breast size goals is to show the doctor what you want to look like using breast implant sizers.

Showing the doctor on your own body what you want to look like is many times better than showing them in photos of other women’s bodies. And it is far better than answering a vague question with a vague answer. If you communicate what you want this way, it will be an answer to every doctor’s approach in breast implant sizing.

By using breast implant sizers you will both avoid the breast implant size discussion as well as the cup size discussion. By showing them what you want, your doctor can have your goal size in mind so that they can choose a breast implant size that will come closest to your goal.

Showing is always better than telling. Seeing your desired breast size goal on your own body in 3-D is far better than seeing it on another woman’s body in a two-dimensional photo.

Determining Breast Cancer Risk Before Elective Plastic Surgery

An article appearing in the January/February issue of the Aesthetic Surgery Journal suggests that patients considering elective breast surgery would gain ultimately improved results after being assessed for breast cancer risk. The article also provides guidelines for plastic surgeons to help women determine breast cancer risk, as well as the most successful options for surgical intervention and breast reconstruction.
There are many risk factors associated with breast cancer, family history probably being the most prevalent. Although these risk factors don’t always mean any woman can or cannot get diagnosed with breast cancer. Estimates predict one out of every eight women will develop breast cancer at some point in her lifetime. That is why it is so important to have regular screenings, either using MRI or mammography to detect the presence of breast cancer, and even genetic testing to determine the risk of developing breast cancer in the future.
“Genetic testing, such as that for BRCA1 and BRCA2 (genes associated with breast cancer), has allowed clinicians to better tailor risk management strategies in patients from families with hereditary breast cancer,” said Hooman Soltanian, MD, lead author of the piece. “Plastic surgeons are in a unique position to provide early preventative options to their patients.”
In Los Angeles cosmetic surgeon Dr. Sean Younai, who is a member of the American Society of Plastic Surgeons and board certified for the American Board of Plastic Surgery also stresses the importance of breast implant safety education especially amongst those at risk for breast cancer.
Early intervention with particularly high risk women can mean they can choose from more surgical and reconstructive procedures.
